List of 2017-2018 Grant Awards:
- CV VFW Post 9601 – Veterans Relief Fund: To provide support to veterans in their time of need. The support will provide basic necessities such as help with rent, medications, food, etc. $5,000.00
- San Leandro Boys & Girls Club – Healthy Choices, Healthy Habits Program: Provides gardening, cooking, and nutrition with a physical fitness regimen to help youth develop healthy lifestyles. This empowers young people to manage their health and to have an impact on their families’ well-being. $10,000.00
- George Mark Children’s House – Pediatric Palliative Care: Their pediatric palliative care program seeks to maximize the health and quality of life for children facing life limiting illness, and to support and empower families to effectively manage this profound challenge. $25,000.00
- CALICO – Creating Connections to Improve Health: The project will improve medical and mental health outcomes for children who have suffered abuse, and their families, by supporting caregivers through the immediate and long-term aftermath of the crisis and ensuring referrals to medical exams are made and that the medical exams occur. $25,000.00
- La Familia Counseling Service – Eden Health Ambassador Program: Provides on-site early intervention and mental health services at the Hayward Center for Education and Careers as well as on-site at the Brenkwitz High School. $20,000.00
- United Seniors of Oakland and Alameda County – Eden Area Senior Action Network: Serves older adults in the Ashland, Cherryland, Castro Valley, and San Lorenzo areas. The program organizes and educates to mobilize allies to improve older adults’ health and quality of life, particularly addressing shrinking resources for seniors at the local and regional levels. Particularly addresses the health concerns of the Ashland and Cherryland communities, focusing on educating and promoting healthy aging. $5,000
- La Clinica de La Raza – Fuente Wellness Center Integrated Health Services for Eden Area Youth: Federally Qualified Health Center that reduces barriers to health care by providing high quality, accessible health services that are culturally and linguistically responsive. Funding supports the provision for youth focused programming at Fuente Wellness Center at the REACH Ashland Youth Center, and critically enhances services for Eden area youth. $25,000.00
- Eden I&R – 2-1-1 Alameda County Communication System: 2-1-1 Alameda County is a free, 24/7 phone service providing callers with information and referrals to health, housing and human services in over 300 languages. This easy to remember three-digit phone number enables individuals and families in need to make a critical link to community based organizations and government agencies. The funds will be used to connect Eden Area residents and their advocates to health related and other service and housing referrals, and continue their pivotal disaster response role. $20,000.00
- East Bay Agency for Children – Child Assault Prevention Program: Provides violence prevention workshops at high-risk San Leandro schools, as well as mental health services and Trauma Awareness Groups. Priority from this funding will be given to the Ashland, Cherryland, and South Hayward areas. $20,000.00
- Restore Women’s Wellness Centers – Vivir la vida al maximo! (Live life to the fullest!): Program which provides mental health services to low-income Latinas who are among the most in need of mental health care and emotional support. Their program will systematically remove barriers to this critical mental health care by providing affordable, accessible, confidential, culturally-sensitive psychotherapy and psychosocial supports groups with Spanish speaking clinicians from diverse backgrounds to the under-served residents of Ashland, Cherryland, and South Hayward, along with community workshops to help erase the stigma surrounding mental health. $20,000.00
- Mercy Retirement & Care Center – Brown Bag Program: Improves the physical and mental health of limited income older adults in Alameda County by coordinating the distribution of nutritious groceries to each household twice a month, free of charge. This model of distribution decreases malnutrition reduces stress, increases exercise and socialization, connects them to other valuable resources, and supports their ability to remain independent in our community, to “age in place”. $25,000.00
- Northern Cal. Society to Prevent Blindness – See Well to Learn Comprehensive Early Vision Intervention for Low Income Underserved Hayward Preschoolers: Provides direct vision screening services, followed by professional optometry exams, and provides no-cost prescription glasses, follow up testing for Hayward Unified School District preschools. $25,000.00
- Meals on Wheels – Volunteers to Keep the Meals Rolling: Meals on Wheels delivers meals to seniors 60 years and older living in Castro Valley, Hayward, San Leandro, San Lorenzo and unincorporated Alameda County. Meal recipients receive a daily home delivered meal, a daily safety check in from their delivery driver, a minimum of two home visits annually, and supplemental food as needed. $25,000.00
