Eden Health District wants to honor members of our community living within Castro Valley, Hayward, San Leandro, San Lorenzo, Fairview, and Ashland/Cherryland areas, who make significant contributions improving the health of people who reside within District boundaries.
Do you know of an unsung hero, someone who goes above and beyond helping people with chronic health conditions like diabetes, heart disease, or cancer…someone who organizes services that promote the health of the community, such as community health clinics or community health education?
